OFFENDER MARRIAGE CHECKLIST (Example)
__________Did you present to the chaplain a notarized
statement stating that there are no legal restrictions
prohibiting the marriage?
__________Did you provide to the chaplain the name, address,
and telephone number of the intended spouse outside minister
and approximate date of marriage?
___________Did you obtain a marriage application and provide
the chaplain with a completed copy and return the original to
your intended spouse?
__________Have you and your intended spouse considered
requesting premarital counseling (optional)?
__________Did you contact an outside minister or authorized
person (optional) and instruct the minister or authorized
person to request approval from the Superintendent/Warden for
him/her to enter the Institution?
__________Did you inform your intended spouse of the policy
concerning dress, rings, camera, etc. and instruct him/her to
Fax or mail a copy of the completed marriage license to the
Director of Chaplaincy in the Central Office?
RETENTION SCHEDULE:
No retention is necessary for this form. It is intended for use by the
inmate/detainee to assure that all preparations on his/her part have been done.
