SUMMARY:
This attachment provides a standardized table that outlines when disciplinary reports (DRs) or disciplinary warning reports should be issued to inmates based on the type of offense and whether it is a first, second, or third or subsequent offense. It applies to all GDC staff responsible for documenting inmate rule violations and establishes progressive discipline escalation for minor infractions while requiring immediate formal disciplinary reports for serious offenses such as assaults, drug possession, and weapons-related violations.

infraction.


**The issuance of a Disciplinary Report, shall be dependent upon the offender’s Mental Health status

at the time of infraction.


Retention schedule: This attachment shall be utilized as instructed in the SOP until it becomes obsolete or revised.
