SUMMARY:
This is an inspection form used to document the condition of cells housing offenders in the Tier II Administrative Segregation Program. Staff inspect specific cell components (lights, doors, locks, walls, windows, beds, toilets, sinks, and other fixtures) and note any discrepancies or damage. Offenders sign the form to acknowledge they understand they may receive a disciplinary report and be charged for any undisclosed damage to state property.
