SUMMARY:
This is a form used to document the 30-day review conducted for juvenile offenders placed in restrictive housing/administrative segregation. The RHA-JOAS Program Classification Committee completes this memo to record their review findings and recommendations regarding the continued placement of the juvenile offender. Both the juvenile offender and staff must acknowledge receipt of the review memo, which is then filed in the offender's case history file.
