PROBATION BOOT CAMP

IN THE SUPERIOR COURT OF COUNTY, GEORGIA

1. The above probationer is remanded to the custody and supervision of a
Georgia Department of Corrections Probation Boot Camp for a period of 120
days. However, the Department may release the probationer no earlier than 90
days in recognition of excellent behavior, and evidence that the offender has
complied with and met all confinement and program requirements imposed by
the Court and the Probation Boot Camp. The probationer is to obey any and
all rules, regulations and policies which are in effect at any time in the future.
During the period of confinement the Department of Corrections may transfer
the probationer to another facility in order to provide needed health care or
other cause essential to the care and supervision of the detainee, or as necessary
for the effective administration and management of its facilities.

2. Jurisdiction of the sentencing Court is transferred to the county wherein the
Probation Boot Camp is located for the entirety of the probationer's custody,
unless the probationer is sentenced under the First Offender Act. Should the
probationer's performance be unsatisfactory at any time during the period of
confinement in a facility, the probationer may be brought before the Court
having jurisdiction for consideration of additional confinement in a probation
facility, revocation of probation or any other disposition the Court deems
necessary. If probation is revoked or any portion thereof, the probationer will
be transferred to the jail of the county from which jurisdiction was transferred,
or the original sentencing county, there to serve the period of revocation or to
await transfer to any other ordered confinement facility or supervision status
as ordered by the Court.

3. Rules, regulations and policies of the Probation Boot Camp will be fully
explained to the probationer. Any noncompliance will be considered sufficient
to warrant disciplinary action up to and including revocation of probation.

4. The probationer shall not ingest, consume or otherwise use any alcoholic
beverage or any substance prohibited or controlled by any law of the State of

Georgia or the United States, except pursuant to physician's prescription;
further, the probationer shall readily submit breath, urine, spittle or blood
specimens for analysis for the presence of a prohibited drug or alcohol.

5. The probationer shall attend and participate in such counseling, treatment or
educational programs as may be directed by Probation Boot Camp staff, and
shall abide by all rules, regulations and directions of any such program. The
probationer may be required to attain a grade five competency level through
participation in education courses while in the center and after release unless
exempted by probation staff as described in Georgia law.

6. The probationer will perform all work tasks to which assigned while at the
Probation Boot Camp, abiding by all instructions, directives and work area
restrictions and boundaries defined by supervising personnel.

7. Limitations on freedom of movement, as defined by Probation Boot Camp
staff, both within the Probation Boot Camp and its perimeter, and while
participating in work and other activities outside the Probation Boot Camp,
will be enforced to the fullest extent allowable by law.

8. Upon successful release, the probationer will arrange for transportation to an
approved residence and shall report within 24 hours to his assigned probation
officer and will participate in and satisfactorily complete the intense
supervision requirements of the Probation Boot Camp program. After the
satisfactory completion of an intense supervision period, the probationer will
continue under the conditions of the sentence as ordered by the Court. The
probationer shall continue participation in any programs as may be directed by
probation staff.
