SUMMARY:
This form is used by the Classification Committee at probation detention centers to evaluate detainees and recommend changes to their supervision level. Staff assess the detainee's work performance, program participation, disciplinary history, and relationships with staff and peers using a scoring system that results in a recommendation to decrease, maintain, or increase the supervision level. The completed form is signed by the Classification Committee and Superintendent, then placed in the detainee's institutional file.
