PROBATION DETENTION CENTER FOREST FIRE RAKE CREW MEMBER AGREEMENT

I,                 , #           ,

do hereby freely and willingly volunteer to participate in the

joint effort of the Georgia Department of Corrections and the

Georgia Forestry Commission in forming detention center forest

fire rake crew teams. By this agreement, I state that I am, to

the best of my knowledge, physically and emotionally capable of

participation and agree to participate in any and all training and

fire fighting activities at my own risk. I agree to hold the

Georgia Department of Corrections, the Georgia Forestry

Commission, and any of their agents, employees or representatives

blameless for injuries I may receive while participating in this

voluntary program. I further agree to complete required training,

and to follow instructions of those in authority while fire

fighting, and to make every effort to respond to requests for

assistance in fighting forest fires. Finally, I understand that I

may voluntarily withdraw from participation at anytime by

initiating and delivering to the superintendent, a signed and

dated statement indicating my withdrawal from participation.
