SUMMARY:
This is the official form used by wardens or superintendents to document their response to offender grievances filed within Georgia Department of Corrections facilities. The form requires the warden/superintendent to provide a written response to the grievance, obtain the offender's acknowledgment of receipt, and inform the offender of their right to appeal the response to the Grievance Coordinator within seven calendar days. Completed forms are retained with the grievance file for four years.
