SUMMARY:
This form is used to document the resolution or withdrawal of formal grievances filed by offenders in GDC facilities. It records whether a grievance has been resolved, dropped by the offender, or discussed between the offender and staff, and requires signatures from both the offender and staff member involved. The completed form is retained with the grievance packet for four years before destruction.
