    "content": "```\n                                    SOP 409.05.04\n                                    ATTACHMENT 1\n                                       8/01/07\n              ATTACHMENT I\n        GEORGIA DEPARTMENT OF CORRECTIONS\n\nVOLUNTARY AGREEMENT OF PIECP PARTICIPATION & WAGE DEDUCTION\n PRISON INDUSTRY ENHANCEMENT CERTIFICATION (\u201cPIECP\u201d) WORK\n                PROGRAM\n\nI _____________________GDC#_________________ hereby certify\nthat I have voluntarily applied for a position in the\n___________________ PIECP Work Program and agree to the\nfollowing conditions of employment:\n\nA. If I am chosen to work in the PIECP Work Program, I\n voluntarily agree to the following deductions and\n distributions from the funds received:\n\n 1. All applicable local, county, state, and federal taxes.\n\n 2. Ten percent of gross wages shall be applied to the\n   Georgia Crime Victim\u2019s Emergency Fund.\n\n 3. If I have court ordered obligation to a victim or\n   restitution, ten percent of my gross wages shall be\n   paid to such orders.\n\n 4. Twenty percent of gross wages will be retained by the\n   Georgia Department of Corrections for room and board.\n\n 5. Twenty percent of gross wages to a saving/reserve\n   account that shall be placed in a saving account in my\n   name for withdrawal upon my release from confinement\n   from the Georgia Department of Corrections.\n\n 6. If I have court ordered child support, all wages going\n   into my account over twenty percent of gross wages and\n   in excess to the above deductions will be used to pay\n   child support.\n\n 7. Withdrawals from Savings/Reserve Account may be\n   considered for PIECP workers who are ineligible for\n   release from confinement and can only be sent to\n   individuals on the PIECP workers approved financial\n   list. PIECP workers must complete the \u201cOffender\n   Miscellaneous Withdrawal form\u201d, in accordance with\n\n```\n\n1\n\n\n```\n                                    SOP 409.05.04\n                                    ATTACHMENT 1\n                                       8/01/07\n 8. Policy 406.19 for consideration. The PIECP coordinator\n   shall review for approval all written request prior to\n   release of funds.\n\n 9. The remainder of gross wages shall be deposited into my\n   account for my personal use while incarcerated.\n\n 10.I will restrict the number of my exemptions claimed\n   for withholding taxes to my verifiable number of\n   authorized dependents and I agree not to request\n   additional tax to be withheld from my pay.\n\n 11.I understand that the above formula is subject to\n   change by the Georgia Department of Corrections through\n   written notification to me.\n\n 12.If I disagree with any changes made to the disposition\n   of wages, I understand that I may discontinue\n   participation in the PIECP work program.\n\n 13.I agree to comply with all the rules of the private\n   employer and the correctional facility in which I am\n   housed.
