Washington State Prison fire inspection, September 24, 2026: padlocks and slide bolts on cell doors

Office of Commissioner of Insurance and Safety Fire, Safety Fire Division · Request S000171-100126 · Filed October 1, 2026 · Partly fulfilled

A State Fire Marshal inspection of Washington State Prison in Davisboro on September 24, 2026 (File 26-014544), made after an anonymous complaint. Inspectors escorted by the warden found slide-bolt latches and padlocks on cell doors in housing units F, G, H and I, and rated the prison Non-Compliant on means of egress: padlocks are not permitted on housing-unit doors or any other door in the means of egress. The warden said the padlocks were used because of broken mortise locks. This record has the three-page report and one photograph of a cell door.

Status: OCI released the September 24 report and one photograph on October 9, 2026. As of October 28 it had not answered the request's other items: advance-notice and scheduling records, the two earlier inspection reports, and 2026 records about the prison's doors.

Records requested under the Georgia Open Records Act (O.C.G.A. § 50-18-70 et seq.), by Georgia Prisoners' Speak or, where a page says so, by another person or group who shared them with GPS. Documents marked "as released" are posted exactly as the agency released them. Files marked "prepared by GPS" are GPS's own compilations of those documents, made for convenience; where they differ, the released document governs. A request is published here when GPS cites it or when its records are useful to readers; records about individual deaths are published on GPS's mortality pages, not here.
