Instructions for taking Convicted Offender DNA Samples

1. Swab inside of cheek using _ALL Four_ buccal swab provided in kit.

3. Seal swab envelope with RED Evidence tape only, _DO NOT MOISTEN ADHESIVE_ to seal.

4. Place DOC submission form and swab envelope into shipping envelope.

5. Seal shipping envelope with Kit Sealing Tape and mail to GBI.
