SOP 227.02-att-2: Staff Local Investigative Report and Recommendation Form
Summary
This is the official form used by GDC staff to document local investigations of offender grievances and complaints. Staff complete the form to record the offender's allegation, summarize their investigation findings, and provide recommendations. The Grievance Coordinator reviews and either concurs or disagrees with the staff findings, and the completed form is retained with the grievance packet for four years before destruction.
