Cause (DC + coroner cert): Cardiorespiratory arrest due to sepsis with multiorgan failure, due to recurrent intra-abdominal abscesses following duodenal perforation and surgical repair, due to peptic ulcer disease.
Contributing: acute respiratory failure with mucus plugging, bilateral pleural effusions, acute kidney injury, chronic anemia, hypertension, GERD.

Pronounced: 4:42 PM 2025-10-25 by Dr. Lajide R. Lawoyin, MD (GA Lic 045892, Internal Medicine / Hospitalist) after 5 cardiopulmonary arrests / 10 min ACLS / 3 doses epinephrine. Documentation electronically signed 4:49 PM EDT.
Clinical: Admitted 2025-10-20 with worsening abdominal pain, confusion, rectal bleeding. Recent (Sept 2025) abdominal surgeries for perforated duodenal ulcer + anastomotic leak. CT confirmed recurrent intra-abdominal abscesses + microperforation + sepsis + AKI + bilateral pleural effusions. Treated with broad-spectrum antibiotics (piperacillin-tazobactam) + supportive ICU care. 2025-10-25: acute respiratory failure from mucus plugging required intubation + bronchoscopy. Repeated cardiac arrests attributable to sepsis + hypoxia + multiorgan failure.
Autopsy: No (GBI ME declined based on available clinical info)
Coroner process anomaly: Coroner Anderson took initial notification and released body from coroner's hold without informing Deputy Coroner Bennett. Bennett later learned of inmate death from a GBI ME contact while working another case; subpoenaed full EGRMC chart and refused to rely solely on GDC internal investigation. Final report prepared by Deputy Coroner Daniel Bennett.
