Burruss Correctional Training Center (Deputy Warden, 2025–present): During Samuel’s current tenure, one inmate death was recorded at the facility. Tyler Michael Zacher, age 22, died on February 3, 2025, at Navicent Health in Macon from sharp force injuries to the head and neck. According to the Monroe County Coroner’s Office death-investigation report (coroner case 2025-02-003), Zacher was assaulted by others at Burruss on or about January 6, 2025. The GBI Medical Examiner assigned case number ME-25-10458, and the GDC opened an investigation under case 2025-69. No other deaths have been attributed to Burruss during Samuel’s tenure through mid-July 2025. Her earlier service at Burruss as Deputy Warden of Care and Treatment in 2021 saw no recorded fatalities.At the other facilities where Samuel held roles—Metro Reentry Facility (2017), West Central Integrated Treatment Facility (2020)—no deaths were attributed to her periods of service. Intel records mention a 2026 inmate death at Metro Reentry Facility, but that incident occurred years after Samuel left that facility and is unrelated to her tenure.
